Dearest patient, 

I would like to thank you for the trust you have given me over the years as your ophthalmologist. Taking care of my patients and their families has been an honor for my staff (Tamika and Lindsay) and me. Your continuing healthcare is a priority for us.

I am writing to inform you that I am retiring from private practice and closing my practice effective as of April 1, 2025. I will be available until that date for your care.

I want to assure you that as I write this, I am both healthy and happy. This decision to retire from private practice did not come easily. Although I have exciting adventures ahead of me, my patients have become my family over the past 25 years I have been in practice. I will miss you all.

It is important that you continue obtaining medical care for your eyes. Therefore, you should establish contact with another physician before April 1, 2025. The following information is provided in order to help you find another doctor:

If you are in a managed care plan, you may need to contact your healthcare insurer for a referral within the network. 

Additionally, we are happy to recommend an ophthalmologist in the metro-Atlanta area. In the next two weeks, our office will start calling patients who have appointments with our office on or after April 1st to assist them in scheduling with a new ophthalmologist. 

If you are currently receiving a prescription for an eye condition, please notify the practice before April 1, 2025. The practice will assist you in being able to fill prescriptions even after the closing.

All medical records are confidential and protected by federal privacy and security regulations. With your authorization, a copy of your medical records can be released to you or transferred to another physician. 

The enclosed HIPAA-compliant authorization form is necessary to release a copy of your medical records to you or your new physician. Please complete the form and return it as soon as possible. On receipt of this signed form, my office will forward a copy of your medical records to you or to the physician you designate. Please rest assured that our office will remain available until April 1, 2025 to consult with your new provider to promote a smooth transition and continuity of care.
